This collaboration began in 2015 and has been continuously developed to provide comprehensive support for children with learning difficulties. In 2023, the program evolved to incorporate modern technological advancements. A screening, consultation, and referral system was integrated into the Care Application, streamlining communication and coordination between schools and hospitals. The objective is to establish a holistic care system, ensuring proper referrals, treatment, and follow-ups for children and adolescents facing learning challenges. Beyond institutional support, the program also aims to equip teachers and parents with knowledge and skills in identifying and addressing learning difficulties and mental health issues. By empowering educators to screen and provide initial assistance, children in need can access timely interventions, promoting life skills development, prevention, treatment, and rehabilitation. Ultimately, this initiative fosters a strong network of teachers, parents, and healthcare professionals, working together to mitigate social issues stemming from learning difficulties. The result is a more inclusive, supportive educational environment where every child has the opportunity to thrive.
